*1. That I am participating in training programs, fitness classes, or other activities, whether private or in a group setting, that are offered by Sara Locklear Burden of Vodou Wellness L.L.C.

*2. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in any fitness program provided by Sara Locklear Burden of Vodou Wellness L.L.C.

*3. If permitted by a physician to participate in any group or private fitness classes, I agree to assume full responsibility for any risks, injuries, or damage, whether known or unknown, which I might incur as a result of participating in the programs or classes o ered by Sara Locklear Burden of Vodou Wellness L.L.C.

*4. In consideration of being permitted to participate in any fitness classes or programs, I knowingly, voluntarily, and expressly waive any claim I may have against Sara Locklear Burden of Vodou Wellness L.L.C and the company Vodou Wellness L.L.C, for injury or damages that I may sustain as a result of participating in any services offered by Sara Locklear Burden of Vodou Wellness L.L.C.

*5. I, my heirs, or legal representatives forever release, waive, discharge, and covenant not to sue Sara Locklear Burden of Vodou Wellness L.L.C for any injuries, claims, disabilities, or death, that I may suffer as a result of negligence or other acts. I have read the above release and waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above.

*Parental Authorization and Release of Liability for participants under 18 years of age:

By completing this field, I, (the parent/guardian) confirm that I have the legal capacity and authority to act on behalf of the minor named above, and waive all liability associated with engaging in services offered by Vodou wellness L.L.C and its instructors.
